Job Title: Director of Marketing/Clinical Liaison

Chatham Hills Subacute Care Center is seeking a highly skilled and experienced Director of Marketing/Clinical Liaison to join our team. This role is responsible for coordinating and enhancing the facility's inquiry, admission, and marketing process.

Key Responsibilities:
  • Takes inquiry calls from hospitals, families, and other healthcare professionals to provide information and answer questions about our services.
  • Conducts clinical reviews of referrals and conducts on-site visits to assess the needs of potential patients and their families.
  • Maintains and grows relationships with hospital case managers, DC planners, and social workers to ensure a smooth referral process.
  • Identifies the needs of patients and families and directs referrals to our facility.
  • Represents our facility at community partner meetings to identify needs and opportunities for growth.
  • Ensures responses to inquiries are handled properly, timely, and with appropriate follow-up.
  • Demonstrates a sense of urgency regarding census and premium mix enhancement and management.
  • Serves as a member of the marketing team to enhance resident and community relations.
  • Assists in maintaining a database of medical contacts and community resources.
  • Assists with bringing physicians on board and maintaining relationships with local ALFs, group homes, and other healthcare providers.
  • Creates new promotional items and ideas to aid in the increase of referrals.
  • Follows up on all hospitalized patients to ensure they return to our facility.
