Director of Marketing/Clinical Liaison Job Summary

Chatham Hills Subacute Care Center is seeking a highly skilled and experienced Director of Marketing/Clinical Liaison to join our team. This key role will be responsible for coordinating and enhancing the facility's inquiry, admission, and marketing process.

Key Responsibilities:
  • Takes inquiry calls from hospitals, families, and other healthcare professionals to provide information and answer questions about our services.
  • Conducts clinical reviews of referrals and conducts on-site visits to assess patient needs and provide recommendations for care.
  • Maintains and grows relationships with hospital case managers, DC planners, and social workers to ensure seamless referrals and admissions.
  • Identifies needs of patients and families and directs referrals to our facility, ensuring timely and effective care.
  • Represents our facility at community partner meetings to identify needs of the target market and promote our services.
  • Ensures responses to inquiries are handled properly, timely, and with appropriate follow-up to maintain a positive reputation and build trust with our community partners.
  • Demonstrates a sense of urgency regarding census and premium mix enhancement and management to drive business growth and revenue.
  • Serves as a member of the marketing team to enhance resident and community relations, promoting our facility's services and values.
  • Assists in maintaining a database of medical contacts and community resources to support referral development and outreach efforts.
  • Assists with bringing physicians on board to promote our services and build relationships with key stakeholders.
  • Maintains and creates relationships with local ALFs, group homes, and other healthcare providers to expand our referral network and promote our services.
  • Develops new promotional items and ideas to increase referrals and drive business growth.
  • Follows up on all hospitalized patients to ensure they return to our facility for continued care.
