Job Summary

We are seeking a highly skilled and compassionate Medical Health Advocate to join our team at Equitas Health. As a Medical Health Advocate, you will play a critical role in providing comprehensive case management services to individuals living with HIV/AIDS, ensuring they receive the necessary support and resources to maintain their health and well-being.

Key Responsibilities
  • Provide case management services to clients, including assessment, planning, implementation, and evaluation of services
  • Develop and maintain relationships with community partners and healthcare providers to ensure seamless coordination of care
  • Assist clients in accessing medical services, health insurance, and other resources to improve health outcomes
  • Conduct comprehensive psychosocial assessments and develop individualized case plans
  • Monitor and evaluate client progress, making adjustments to case plans as needed
  • Collaborate with multidisciplinary teams to ensure comprehensive care and support
  • Maintain accurate and timely documentation of client interactions and services provided
  • Participate in ongoing training and professional development to enhance skills and knowledge
Requirements
  • Master's degree in Social Work or related field
  • Minimum of 2 years of experience in case management or a related field
  • Strong knowledge of HIV/AIDS, healthcare systems, and community resources
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Proficiency in Microsoft Office and other computer applications
