Overview

The Integrated Care Coordinator plays a vital role in our outreach initiatives, managing a diverse caseload of consumers. This position requires flexibility to meet consumers in various settings, including their homes and community locations, while adhering to regulatory guidelines based on individual care levels.

Key Responsibilities
  • Consumer Engagement: Develop and implement personalized service plans in collaboration with consumers, focusing on their strengths and needs to foster growth and community integration.
  • Service Navigation: Assist consumers in identifying and accessing necessary services outlined in their service plans, ensuring support is provided where it is most needed.
  • Monitoring and Advocacy: Regularly review the services provided to consumers, addressing any challenges they face and advocating on their behalf when necessary.
  • Support Network Development: Facilitate connections with family members and informal support networks, with the consumer's consent, to enhance their support system.
  • Resource Utilization: Guide consumers in identifying community resources and assessing their ability to utilize these resources effectively.
Documentation and Compliance
  • Maintain comprehensive knowledge of documentation requirements and ensure timely completion of all necessary paperwork.
  • Adhere to all organizational policies and procedures regarding documentation and case management.
  • Stay informed about relevant federal, state, and agency regulations to ensure compliance in all case management activities.
Training and Development
  • Complete mandatory training programs within the initial weeks of employment and participate in ongoing training every two years.
  • Engage in regular supervision sessions to discuss progress and address any concerns.
  • Exhibit strong interpersonal skills to effectively interact with consumers and colleagues.
Crisis Management

As the primary support contact during crises, the Integrated Care Coordinator is responsible for providing 24/7 on-call services, collaborating with other team members to ensure comprehensive care.

Performance Expectations

Consistently meet established productivity metrics while communicating any barriers encountered to supervisory staff.

Qualifications
  • A Bachelor’s degree in a relevant field or equivalent experience in mental health services.
  • Clearances and training as required by law.
  • Proficient computer skills and the ability to work in various environments.
  • Strong organizational and time-management abilities.
  • Ability to maintain composure in high-stress situations and effectively manage crisis interventions.
  • Valid driver's license and appropriate insurance coverage.
