About the RoleThe Admissions Nurse is responsible for executing the hospice admission process in compliance with federal and state regulations. This role requires strong communication and interpersonal skills, as well as the ability to work effectively with patients, families, and healthcare professionals.
Key Responsibilities- Assess patient needs and eligibility for hospice care
- Develop and implement individualized care plans
- Communicate effectively with patients, families, and healthcare professionals
- Maintain accurate and confidential patient records
- Collaborate with the interdisciplinary team to ensure high-quality patient care
- Current RN licensure in the state of operation
- At least 2 years of experience in a hospice, home health, or other healthcare setting
- Current CPR certification
- Reliable transportation and a valid driver's license
