Home Health RN Case Manager

We are seeking a skilled and compassionate Registered Nurse Case Manager to join our team at Preceptor Home Health & Hospice. As a key member of our home health and hospice team, you will play a vital role in delivering high-quality patient care and support to individuals in their own homes.
Job Summary:As a Home Health RN Case Manager, you will be responsible for assessing patient needs, developing and implementing individualized care plans, and coordinating with healthcare providers to ensure seamless care transitions. Your exceptional communication and interpersonal skills will enable you to build strong relationships with patients, families, and healthcare teams.
Responsibilities:- Conduct thorough patient assessments to identify care needs and develop personalized care plans
- Collaborate with healthcare providers to ensure timely and effective care delivery
- Coordinate with patients, families, and caregivers to ensure smooth care transitions
- Monitor patient progress and adjust care plans as needed
- Provide education and support to patients and families on disease management and self-care
- Current licensure as a Registered Nurse in the State of Wisconsin
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Good organizational and time management skills
- Willingness to travel to meet patient needs
