The

Living Assistance Program Director


oversees the health and wellness needs of each resident in the assisted living program, including resident evaluations, medication management, and support with daily living activities.

Responsible for hiring and supervising nursing personnel within the assisted living unit.


Ensures that all services are delivered with a focus on the health, safety, and satisfaction of our residents.


Key Responsibilities:


Collaborates with the Director of Post-Acute Services and/or Director of Health Services regarding the daily operations of clinical services across the campus.

Ensures that pre-admission assessments for each resident are completed prior to their move-in date. Regularly monitors residents for any changes in condition to ensure timely interventions.

Leads the Lifestyle and Care Needs Assessments to identify the initial and ongoing medical and personal care requirements of residents.

Assists residents with Activities of Daily Living (ADL), which may include bathing, dressing, grooming, and dining.
Ensures that CareTracker information is updated promptly and that all documentation is completed in a timely manner.
Directs and verifies that documentation is accurate and that any changes in resident needs are addressed.

Facilitates the Assisted Living Resident Meeting and ensures that feedback and concerns are addressed promptly.

Coordinates all pharmacy and medication needs while ensuring compliance with state regulations and company policies.

Acts as a liaison to foster communication between residents, the health campus, external healthcare providers, family members, and referral sources to effectively meet the health-related needs of residents.

Supervises and participates in the execution of treatment plans as per physician orders.
Ensures that resident medical records and chart information are maintained in accordance with state guidelines.

Assists in the training of Nursing Assistants (CNA, QMA, etc.) in providing daily living assistance, medication policies, infection control, dietary concerns, and other health-related training necessary to meet residents' needs and comply with state regulations.

Provides support in the recruitment and selection process for Nursing Assistants and licensed staff within the assisted living unit.
Collaborates with the Director of Post-Acute Services and/or Director of Health Services in the hiring, recruitment, and ongoing training of clinical staff.
Ensures that all staff meet the necessary CPR/First Aid training requirements as per state regulations and company policies.
Guarantees that all orientation and training requirements are fulfilled as mandated by state regulations and company policies.
Serves as weekend management and provides coverage and on-call duties on a rotating basis or as needed.
Assists in organizing and directing marketing initiatives for the campus in alignment with company policies.
Implements strategies to cultivate relationships with key referral sources.
Distributes campus information to referral sources.
Acts as a point of contact for inquiries, tours, and move-in procedures when necessary.
Participates in various committees of the health campus (e.g., infection control, policy advisory, quality assurance) as required and appointed by the Executive Director.
Engages in annual company-mandated in-service training programs.
Minimal travel may be required, including overnight stays as necessary.

WHAT WE'RE LOOKING FOR:
LPN with a degree from an accredited nursing program.

Licenses/Certifications:
Current unencumbered LPN license in the state of practice.
Valid CPR certification.
Valid driver's license and ability to operate a large van/bus (up to 15-passenger) vehicle.
