Job Summary

This position will work 5 days each week in 8-hour shifts. The Resource Nurse will complete hospice admissions for new patients and assist with visits throughout the week. Admissions occur in patient homes including private residences, assisted living communities and skilled nursing facilities. This position is a contributing member of the interdisciplinary team, providing expertise regarding psych-social assessments, interventions, and knowledge of community sources.

Responsibilities
  • Complete hospice admissions for new patients
  • Assist with visits throughout the week
  • Work closely with patients on a 1-1 level but get strong support from a patient care team
  • Manage visits, documentation, meetings, and respond to urgent needs by prioritizing and triaging multiple concerns
  • Ability to learn an electronic medical records system
  • Must be willing to drive with reliable transportation, valid drivers license, and auto insurance, to be able to travel across the service area
Requirements
  • Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice
  • Works well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership
  • Time management skills to manage visits, documentation, meetings, and respond to urgent needs
