Job Title: Licensed Social Worker Foster Care

Job Summary:
New Directions Youth & Family Services, Inc. is seeking a skilled Licensed Social Worker to provide per diem support to families and children in need. As a Licensed Social Worker Foster Care Specialist, you will work up to 25 hours a week, coordinating with an interdisciplinary team to deliver individualized care plans and support. This role is ideal for someone with experience in social work, who is adaptable, good with people from all walks of life, and ready to make a real impact.

Responsibilities:
1. Complete FASP's, 29-i assessments, Connections notes, Treatment Plans, and Adoption paperwork.
2. Ensure services are responsive to the family vision and needs identified in the FASP and Treatment Plan.
3. Work virtually with an in-person service provider to complete sessions, assessments, and consultations.
4. Seek community resources for the family, referring them to community resources and other organizations as needed.
5. Empower families to manage their own lives post-discharge, teaching them skills to marshal resources and advocate for themselves.
6. Maintain and manage case records in accordance with Department of Social Services and Family Court guidelines.
7. Provide treatment as indicated in the Service plan, including family and individual work.
8. Report to referral agencies, including Family Courts, and attend court hearings when necessary.
9. Participate in team meetings to coordinate services for families and youth.
10. Facilitate referrals for on-going services and provide these services as appropriate.

Requirements:
• LMSW Social Work or related field, and one year of experience working with youth/families at risk.
• Experience in the provision of individual, family, and group services to adolescents and their families.
• Commitment to development of community resources and informal supports designed to meet the needs of families within their own cultural and community context.
