Position Overview: As a vital member of the healthcare team, you will deliver integrated, patient-focused nutritional care and education aimed at fostering behavior change and enhancing overall well-being. This role emphasizes support and advocacy for individuals impacted by diabetes, facilitating optimal health outcomes through personalized self-management strategies.

Key Responsibilities:

  • Conduct thorough evaluations, analyze individual health information, and create tailored care plans that align with the patient's specific needs and objectives, promoting effective self-management.
  • Maintain accurate documentation in the Electronic Health Record and outcomes database, adhering to established guidelines and timelines.
  • Engage collaboratively with fellow diabetes care professionals to formulate individualized diabetes management plans.
  • Promote and support the use of technology-driven medical nutrition therapy and services for both individual and community health.
  • Work alongside community partners to develop and sustain ongoing support resources.
  • Contribute to the achievement of departmental goals while complying with policies, procedures, and quality standards, including governmental and accreditation regulations.
  • Adhere to the ADCES Diabetes Education Accreditation Program requirements, which align with the National Standards for Diabetes Self-Management and Support.
  • Participate in team meetings, serve on committees, and represent the department in community outreach initiatives as necessary.
  • Perform additional duties as assigned.
Qualifications:
  • Possess a Bachelor’s degree in dietetics or nutrition, with a valid license registered by the Commission on Dietetic Registration.
  • One year of experience in patient care is preferred.
  • Additional training in chronic disease management or related healthcare fields is advantageous.
Skills and Competencies:
  • In-depth knowledge of nutritional guidelines for chronic disease management and educational strategies for disease self-management.
  • Ability to manage programs effectively and analyze data to enhance quality outcomes.
  • Proficiency in various software applications, including spreadsheets, along with strong organizational skills to prioritize and manage multiple tasks under tight deadlines.
  • Excellent written and verbal communication abilities.
  • Commitment to maintaining professional integrity and personal ethics.
  • Bilingual proficiency in English and Chinese (Cantonese, Mandarin, or Fuzhou) is preferred.
