Job SummaryThe Behavioral Health Patient Coordinator will play a critical role in providing exceptional support to individuals and families seeking behavioral health services. This dynamic professional will serve as the primary point of contact, ensuring seamless communication and coordination between patients, families, and healthcare providers.
Key Responsibilities- Implement a comprehensive screening and triage process to identify the most effective services for each individual.
- Provide empathetic and informative support to patients and families, addressing their concerns and needs in a timely and professional manner.
- Maintain accurate and confidential records, ensuring seamless communication and coordination between patients, families, and healthcare providers.
- Collaborate with clinical staff to develop and implement personalized care plans, addressing the unique needs of each individual.
- Ensure timely and efficient scheduling of appointments, services, and interventions, minimizing wait times and maximizing patient satisfaction.
- Develop and maintain strong relationships with patients, families, and healthcare providers, fostering a culture of trust, respect, and open communication.
- Bachelor's degree in a related field (e.g., social work, psychology, healthcare administration) or equivalent experience.
- 2+ years of experience in a behavioral health setting, with a focus on patient coordination and support.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
- Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities.
- Proficiency in Microsoft Office Suite and electronic health record systems.
- Ability to maintain confidentiality and handle sensitive information with discretion.
