Job Summary

SSM Health is seeking a skilled Licensed Practical Nurse (LPN) to join our Family Medicine team. As an LPN, you will play a vital role in providing high-quality patient care and contributing to the success of our healthcare organization.

Key Responsibilities
  • Contribute to patient assessments and ensure the well-being of patients.
  • Administer medications and monitor patients for adverse reactions.
  • Perform laboratory tests and therapeutic services.
  • Educate patients and families about disease treatment plans, including self-care post-discharge and follow-up care.
  • Document patient encounters, including diagnoses, symptoms, and discharge instructions.
  • Transcribe patient orders, including laboratory tests, radiology tests, and medications.
  • Transcribe consultations and discussions with family members and private physicians.
  • Work in a safe and alert manner.
Requirements
  • Graduate of an accredited school of nursing or equivalent education for licensing.
  • No experience required.
Physical Requirements
  • Constant use of speech to share information through oral communication.
  • Constant standing and walking.
  • Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs.
  • Frequent sitting, reaching, and keyboard use/data entry.
  • Frequent use of vision and depth perception for distances near and far.
  • Frequent use of smell to detect/recognize odors.
  • Frequent use of hearing to receive oral communication.
  • Occasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs.
  • Occasional lifting/moving of patients.
  • Occasional bending, stooping, kneeling, squatting, twisting, gripping, and repetitive foot/leg and hand/arm movements.
  • Occasional driving.
  • Rare crawling and running.
Professional Licenses and Certifications
  • Basic Life Support Healthcare Provider (BLS HCP) - American Heart Association (AHA).
  • Licensed Practical Nurse (LPN) - Illinois Department of Financial and Professional Regulation (IDFPR).
