Hospice Care Coordinator – Cobb County
We are collaborating with a forward-thinking, well-managed, and financially sound hospice organization in the Cobb County region of Georgia. This nurse-owned and operated entity is dedicated to ensuring patient satisfaction, delivering clinical excellence, and providing quality care to every family member.
Our client offers the following benefits:
- Annual Salary: $80,000-$90,000 +
- Health Insurance - Employer Pays % COBRA
- Dental and Vision Insurance – Employer Pays % COBRA
- Mileage Reimbursement - $0.48 Per Mile
- PTO - 14 Days
- Paid Holidays - 7 Federal
- 401K with 3% Match
Key Responsibilities:
- The RN Case Manager will oversee a caseload of approximately 12-14 patients.
- Experience with MatrixCare is essential.
- Average patient visits per day: 4.
- The RN Case Manager will be accountable for backup call duties.
Qualifications:
- Registered Nurse licensed in the State of Georgia.
- 2-3 years of experience in hospice or palliative care is preferred.
- A minimum of 2 years of in-home patient care experience is required.
- Familiarity with the EMR system MatrixCare is preferred.
