Job Summary:

As a Licensed Practical Nurse (LPN) in our Clinic I (Meds) team, you will provide direct and indirect nursing care to patients in an outpatient setting. Under the direction of a Registered Nurse and medical orders from a Practitioner, you will be responsible for reviewing and preparing medical records, initiating follow-up for medical records, and identifying and preparing orders for patient needs. You will also be responsible for observing, collecting, and documenting new information on patients, informing RNs or Practitioners regarding incomplete data, new observations, or abnormal/unusual data.

Key Responsibilities:
  • Review and prepare medical records for assigned patients, initiate follow-up for medical records as requested, and review patients' needs to identify and prepare orders for patient needs.
  • Implement basic nursing procedures per plan of care, administer treatments, immunizations, and medications as prescribed by Practitioners, prepare medications for Practitioners to use, perform point-of-care testing as authorized, and provide laboratory, x-ray, and other reports to Practitioners for review.
  • Document implementation of orders/interventions, record findings, instructions, and patient responses on appropriate forms or online records using department and/or JCAHO standards, observe patients/families' responses to the plan of care, report unexpected responses to RNs or Practitioners, and collaborate with RNs/Practitioners to revise care plans when needed.
  • Demonstrate and apply competency in basic nursing skills and use of equipment, maintain current knowledge and skills needed to perform the position, attend and participate in staff meetings as needed, and participate in projects as requested.
  • Perform efficiently in emergency patient care/clinic situations, follow established procedures, inform appropriate Practitioners/RNs, and assign basic nursing tasks to unlicensed personnel as appropriate and according to department/unit standards.
  • Follow discharge plans developed by Practitioners or RNs, process and/or follow-up with referrals as directed by Practitioners, provide instruction and pre-printed information to patients as directed by RNs and/or Practitioners, observe and document patient/family responses to instructions, and report any concerns.
  • Manage all patient vaccine data, including updating the Hawaii Immunization Registry System (HIR), Electronic Health Records (EHR), and all patient and/or external records at each patient visit, ready any available immunization information prior to the time of visit with the provider, store and administer immunizations according to Health Plan and VFC guidelines, identify patient immunization requirements in accordance with age-specific guidelines per CDC, fill out appropriate immunization paperwork for recording purposes, facilitate vaccine Health Plan and VFC inventory control at assigned clinics, order vaccine and maintain appropriate vaccine inventories, utilize file maintenance to ensure newly arrived vaccines are entered in the EHR lot manager system as needed, assure immunization clinics comply per Health Plan, federal, and state guidelines, complete annual VFC enrollment training requirements and forms, and maintain all records related to the VFC program per program guidelines.
  • Train new and existing LPNs on emergency vaccine storage procedures, serve as a liaison with the state to coordinate VFC (vaccines for children) inventory, generate reports on patients and contact them for immunization compliance and updates as needed, assist in ensuring compliance with infection control policies and protocols, follow medication administration, storage procedures, Health Plan CDC, State, and federal VFC program guidelines, provide educational materials to patients and review printed instructions with patients, collaborate with RNs/Practitioners to revise teaching plans when needed, participate in orientation of new personnel, orient and serve as a resource to LPNs and unlicensed personnel as needed, demonstrate and instruct other team members in use of vendor-specific products and/or department/unit standards and procedures, demonstrate sufficient knowledge, skills, and abilities in the area of specialization, perform clean/sterile technique, set up for procedures, assist with clinical procedures within department scope, follow the recommended process for handling point-of-use instruments as outlined by the High-Level Disinfection Committee, maintain, order, organize, rotate, and stock supplies, schedule patients for routine care, acute care, and addressing care gaps, prepare and process patient forms, and perform other duties and accept responsibility as assigned within scope.
Basic Qualifications:
  • Twelve (12) months (2080 hours) LPN experience.
Education:
  • Hawaii State Board of Nursing approved curriculum for LPN licensure.
License, Certification, Registration:
  • Licensed Practical Nurse (Hawaii) required at hire.
  • Basic Life Support from American Heart Association.
Additional Requirements:

  • Talking to co-workers, customers, outside vendors, and on the telephone.
