Clinical Case Manager

As a Case Manager & Care Coordinator at Kaiser Permanente, you will play a vital role in providing high-quality patient care and ensuring seamless transitions between healthcare settings. This is an exciting opportunity to make a meaningful impact on the lives of our patients and their families.
Key Responsibilities:- Develop and implement effective case management plans to meet the unique needs of each patient.
- Collaborate with healthcare teams to coordinate care and ensure timely discharges.
- Provide education and support to patients and their families to promote healthy behaviors and self-management.
- Identify and address potential barriers to care, such as social determinants and access to resources.
- Participate in quality improvement initiatives to enhance patient outcomes and experiences.
- Minimum one year of experience working with low-income/high-risk patients in a healthcare setting.
- Bachelor's degree in nursing or four years of experience in a directly related field.
- Registered Nurse License (Hawaii) required at hire.
- Basic Life Support from American Heart Association.
- Case management/community outreach experience.
- Program development and operations-level implementation.
- Geriatric experience in outpatient clinic settings.
- Supervisory/lead experience.
