Job Title: Children's Care Coordinator

The New Horizon Counseling Center is seeking a dedicated and compassionate Children's Care Coordinator to join our team. As a key member of our interdisciplinary team, you will provide support to families and children with serious emotional disturbance, chronic conditions, or trauma experiences.

Key Responsibilities:
  • Assess and evaluate the needs of each case to ensure the provision of necessary services.
  • Coordinate and monitor all services, including medical, mental health, and educational wellness/success, through collaborative relationships.
  • Provide counseling, crisis intervention, referral, and advocacy services, as well as assistance with basic needs, through home and field visits.
  • Develop and implement written care plans and crisis plans, as needed, in collaboration with each child and family.
  • Prepare and maintain accurate charting, assessment forms, and accountability forms, ensuring compliance with state and city regulations.
  • Attend regular supervisory and case conference sessions, trainings, and other meetings, as assigned.
Requirements:
  • Bachelor's Degree Required; Master's Degree Preferred in a field such as Social Work, Psychology, Education, or Human Services.
  • Training and successful certification as a CANS-NY Assessor within 30 days of start date.
  • Two years of related human services experience for B.A. level candidates, or one year of experience for M.A. level candidates, in providing direct services or linking children with serious emotional disturbances to services.
  • Excellent interpersonal, communication, and time management skills, as well as English-language writing skills necessary for record keeping.
  • A team player with creativity, commitment, and initiative to be part of a growing, dynamic program.
Preferred Qualifications:
  • Knowledge of Community Resources, Medicaid, and other entitlements.
  • Knowledge of Trauma, Child Welfare, and/or Child Development.
  • Bilingual ability.
