Licensed Practical Nurse

The primary objective of this role is to deliver direct skilled nursing, intervention, and supportive care activities within an inpatient setting, adhering to the standards set by Hospice Satilla and upholding its reputation as a Center of Excellence.
Key Responsibilities
Nursing Duties
Exhibit a commitment to Hospice Satilla's collaborative approach to patient care, which encompasses:
- Conducting assessments, developing plans, implementing care, and evaluating outcomes as part of the nursing process.
- Providing skilled nursing assessments and hospice/palliative care to enhance the comfort and health of patients and their families in line with Hospice Satilla's standards.
Maintain and update patient records to ensure that problems, plans, actions, and goals are clearly articulated, with changes documented as they occur.
Oversee the physical care of patients by instructing primary caregivers, volunteers, and employed caregivers.
Respond to medical or emotional crises and the death of patients, providing direct care as needed.
Demonstrate knowledge of agency guidelines and the regulations of community, state, and federal bodies that pertain to hospice service delivery. Actively participate in patient care conferences.
Accept additional assignments as necessary.
Receive patient care plans for the appropriate discipline and execute the program with minimal direct supervision. The Director of Nursing will provide direct clinical oversight, while the Executive Director will offer non-clinical supervision as required. Adhere to Universal Precautions guidelines when delivering patient care.
Assist in the maintenance of medical records, ensuring accuracy, completeness, and compliance with licensing and certification regulations as needed.
Contribute to building and sustaining Hospice Satilla's reputation as a premier hospice care provider by acting as a professional member of the staff. This includes consistently demonstrating professional standards as outlined in the Hospice Satilla Associate Handbook and as communicated by management. Foster a positive work environment by sharing information, employing problem-solving techniques, and being open to new ideas, feedback, or constructive criticism from colleagues.
Ensure a safe environment for patients, guests, visitors, and staff.
Comply with all policies, procedures, and regulations concerning patient care, conduct, safety, fire, security, and risk management.
