Job Title: Lead Clinician for Assertive Community Treatment Team

We are seeking a highly skilled and experienced Lead Clinician to join our Assertive Community Treatment (ACT) team at HealthPort. As a key member of our interdisciplinary team, you will provide high-quality treatment using evidence-based practices to clients living in the community struggling with behavioral health issues.

Job Responsibilities:
  • Provide individual and group therapy to clients using ACT evidence-based practices.
  • Complete all regulatory documentation for client treatment, including progress notes and treatment plans.
  • Collaborate with the treatment team to develop and implement individualized treatment plans.
  • Coordinate treatment and stabilization with identified natural supports and community service providers.
Qualifications:
  • Current licensed mental health professional with a Maryland license (LGPC, LCPC, LMSW, or LCSW-C).
  • Highly developed organizational and communication skills.
  • Current CPR and First Aid certification.
  • Valid driver's license and safe driving record.
