Eliot's Behavioral Health Division seeks an experienced Care Transitions Coordinator to join their team. This role offers an exciting opportunity to make a meaningful impact on the lives of individuals with complex medical, behavioral health, and social needs.

As a key member of the Care Transitions Team, you will work collaboratively with Nurse Care Managers to analyze hospital admission data, develop strategic plans, and ensure effective care transitions. Your expertise in Care Management or Case Management will be instrumental in empowering individuals to take control of their care.

Main Responsibilities:
• Collaborate with Nurse Care Managers to develop and implement follow-up strategies for hospitalized enrollees
• Facilitate comprehensive discharge planning processes for enrolled individuals
• Ensure timely face-to-face interactions with enrollees in the community following inpatient discharge

Salary Information:
Annual salary: $50,000; bilingual differential available for candidates fluent in Spanish (Bilingual Salary: $53,000 annually)
