Stroke Navigator RN Lead

This is an exceptional opportunity to work as a Stroke Nurse Navigator, RN at Lowell General Hospital. The successful candidate will have the chance to lead and coordinate multidisciplinary teams across the continuum, ensuring optimal transitions of care for stroke patients.
Job OverviewThe Stroke Nurse Navigator reports directly to the Stroke Program Manager, working closely with the Director of the Stroke Center. As a key member of the team, you will serve as a point of contact for patients and families, coordinating care and facilitating effective communication between healthcare providers.
Responsibilities:- Perform daily rounds on inpatients with stroke as a primary diagnosis
- Supports the stroke program and maintains regulatory compliance with state and national standards for designated stroke centers
- Collaborates with medical and hospital staff to provide multidisciplinary care to stroke patients
- Develops program plans, policies, protocols, and pathways to support timely and effective care for stroke patients
- Collects and analyzes stroke program patient data to support documentation, clinical care, and improved outcomes
- Collaborates with stakeholders to develop educational components and teaching material for staff, patients, and the community
- Ensures protocols and therapies become the standard of care followed within the facility
To be considered for this role, you should possess:
- An Associate's degree in Nursing
- A Registered Nurse (RN) license
- Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certification
- Three (3) years clinical experience caring for patients with complex medical diagnoses
Lowell General Hospital offers a competitive salary of $83,000 - $110,000 per year, depending on experience. Additionally, we provide:
