Hybrid RN, Diabetes Management Specialist

We are seeking a full-time, 40-hour Registered Nurse to support our practice in Medford, MA. The ideal candidate will have experience in diabetes education and management, as well as a strong understanding of endocrinology.
The Hybrid RN will work on-site 4 days a week and remotely 1 day a week, conducting health assessments and providing diabetes education to patients. The successful candidate will work closely with our two endocrinologists and be responsible for troubleshooting concerns related to blood sugar, diabetes medications, and technology such as continuous glucose monitors and pumps.
Responsibilities- Conduct health assessments and provide diabetes education to patients
- Assist patients in monitoring blood glucose readings and patterns
- Perform patient care procedures within the RN scope of practice, including injections, blood draws, and EKGs
- Assist in lab specimen preparation and follow up on results as needed
- Coordinate outreach for quality measures and identify patients who have not met quality measures
- Graduate of an accredited Nursing program
- Licensed to practice as an RN or NP in the state of Massachusetts
- Certified Diabetes Care and Education Specialist (CDCES) preferred
- Minimum of one year of experience in insulin and glucometer education and one year of pump and sensor (CGM) education
