RN - Home Health Care Specialist

Providence Home and Community Care (HCC) is seeking a skilled Registered Nurse (RN) to join our team as a Home Health Care Specialist. As a Home Health Nurse, you will provide high-quality patient care in the comfort of patients' homes, working collaboratively with our interdisciplinary team to deliver comprehensive care.
Key Responsibilities- Provide direct patient care, including assessments, care planning, and implementation of individualized care plans.
- Collaborate with the healthcare team to coordinate patient care activities and ensure seamless transitions between care settings.
- Uphold the standards of confidentiality, attendance, and punctuality, maintaining a professional demeanor at all times.
- Accept professional development as an integral part of nursing practice, staying up-to-date with the latest evidence-based practices and guidelines.
- Education to meet certification, license, or registration requirements.
- Current Oregon Registered Nurse License.
- National Provider BLS - American Heart Association certification within 30 days of hire.
- 1 year of Medical/Surgical experience in an acute care setting required for Home Health RN.
- 2 years of Pediatric experience in an acute care setting for RN caring for pediatric patients.
