RN - Home Health Specialist

We are seeking a skilled Registered Nurse to join our Home Health team at Providence. As a Home Health Nurse, you will provide high-quality patient care in the comfort of their own homes, working collaboratively with our interdisciplinary team to deliver comprehensive care.
Responsibilities- Assess patient needs and develop individualized care plans to meet their unique requirements.
- Provide direct patient care, including wound care, medication management, and education on disease management and self-care.
- Collaborate with other healthcare professionals to ensure seamless care transitions and coordination.
- Document patient progress and outcomes, maintaining accurate and timely records.
- Current Oregon Registered Nurse License.
- 1 year of Medical/surgical experience in an acute care setting required for Home Health RN.
- 2 years of Pediatric experience in an acute care setting for RN caring for pediatric patient.
