Care Manager RN

We are seeking a skilled RN Case Manager to join our team at Providence Medford Medical Center. As a key member of our care management team, you will be responsible for developing and managing patient care outcomes for an entire patient case load.
Key Responsibilities- Perform assessment, treatment, or care for patients of all ages, including neonate, pediatric, adolescent, adult, and geriatric, based on population focus.
- Develop and implement complex discharge plans and utilization reviews to ensure effective use of resources and meet clinical and financial outcomes.
- Collaborate with patients, families, assigned nurses, and healthcare providers to achieve patient outcomes.
- Provide expert professional registered nursing care to patients, families, and communities, promoting health, preventing disease, and alleviating suffering.
- Oregon Registered Nurse License
- Three years of acute care hospital experience
- One year of experience in home health, mental health, substance use, hospice, and/or utilization review
- Progressive nursing leadership experience, including charge nurse role
