Position Overview:
As a Patient Care Assistant, you will engage in hands-on nursing care, focusing on daily living needs and personal hygiene.

Key Responsibilities:
- Monitor the health status of individuals and ensure the delivery of quality services.
- Assist and supervise individuals in daily living activities, personal care, and community integration.
- Maintain a safe and supportive environment for those we serve.
- Initiate emergency protocols as necessary.
- Utilize equipment safely and correctly.
- Report any hazardous conditions or incidents to your supervisor.
- Keep accurate progress notes and other relevant documentation.
- Support resident activity programs, including outings and appointments.
- Assist with household tasks and meal preparation.
- Prepare rooms for new residents and assist with discharge processes.
- Actively participate in team meetings and training sessions.
- Uphold confidentiality and treat all individuals with dignity and respect.

Qualifications:
- Completion of a CNA certification program; current CNA certification from the relevant state.
- Preferred one year of experience as a CNA, particularly with medically complex patients.
- Current certifications in Basic Life Support (BLS), CPR, and First Aid.
- Strong communication skills with staff, residents, and families.
- A reliable and compassionate approach to care.
- A commitment to delivering quality services.
