Home Health Nurse

We are seeking a skilled and compassionate Home Health Nurse to join our team at South Shore Health. As a key member of our community health team, you will provide high-quality nursing care to patients in their homes, promoting independence, dignity, and overall well-being.
Key Responsibilities- Scheduling and Visits: Create and manage weekly schedules, prioritizing nursing visits based on client acuity and organizational emergency preparedness plans.
- Assessment and Documentation: Conduct thorough admission assessments, developing individualized plans of care in collaboration with patients, families, and caregivers.
- Reassessment and Visit Documentation: Implement and update plans of care, ensuring timely and effective interventions, and documenting patient progress and outcomes.
- Discharge and Documentation: Ensure smooth transitions to discharge, documenting patient progress and outcomes, and identifying variances or areas for improvement.
- Medication Management: Administer and document medications, ensuring accurate reconciliation and profile updates, and educating patients and families on medication management.
- Case Management: Coordinate patient care, involving patients and families in the planning process, and communicating with interdisciplinary teams to ensure quality care.
- Safety Awareness: Foster a culture of safety, adhering to hand hygiene and infection control guidelines, and promoting a safe environment for patients and staff.
- Professional Development: Engage in opportunities for professional growth, incorporating evidence-based practice and research into clinical practice.
- Leadership Development: Assume leadership roles, demonstrating advanced clinical skills and knowledge, and promoting a positive image of the professional nurse.
Requirements
- Graduate of an accredited School of Nursing
- Bachelor of Science degree in nursing from a college/university accredited by the NLN (preferred)
- One to two (1–2) years of medical/surgical nursing experience (preferred)
- Basic Nursing competencies
- Ability to write, speak, and understand English
