  • Collaborate with patients, families, and healthcare teams to ensure seamless care coordination and communication.
  • Administer medications, perform treatments, and provide education to patients and caregivers on disease management and self-care techniques.
  • Document patient progress, identifying areas for improvement and implementing changes to enhance care quality.
Requirements
  • Graduate of an accredited School of Nursing with a Bachelor of Science degree in nursing preferred.
  • Minimum one to two years of medical/surgical nursing experience, with a focus on home care or community health.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse patient populations and healthcare teams.
  • Flexibility to work a variety of shifts, including days, evenings, and weekends, with occasional on-call responsibilities.
