Job Summary

We are seeking a skilled Home Care Practitioner to join our team at South Shore Health. As a key member of our community health team, you will provide high-quality nursing care to patients in their homes and facilitate the admission process for VNA services.

Key Responsibilities
  • Scheduling and Prioritization

Develop and implement daily schedules for VNA service admissions, prioritizing patients based on acuity and organizational emergency preparedness plans.

  • Assessment and Documentation

Conduct thorough admission physical and psycho-social assessments to determine individualized plans of care for patients, collaborating with patients, families, and caregivers as needed.

  • Reassessment and Visit Documentation

Implement and update plans of care, nursing interventions, and procedures necessary for skilled patient care, as evidenced by chart review and observation.

  • Discharge and Documentation

Ensure smooth patient discharges, documenting improvements in outcomes or identifying variances as necessary, and preparing patients for discharge with all involved disciplines in agreement.

  • Medication Management

Administer and document medication reconciliation, review medication profiles, and teach patients and families about medication management, including side effects.

  • Case Management

Coordinate patient plans of care, involving patients and families in the planning process, and communicating with all disciplines to ensure quality care.

  • Safety Awareness

Foster a culture of safety through personal ownership and commitment to a safe environment, adhering to current hand hygiene and infection control guidelines.

  • Professional Development

Participate in opportunities to maintain, enhance, and promote the image of the professional nurse, responding to and participating in opportunities for professional growth.

  • Leadership Development

Serve in formal or informal leadership roles, demonstrating leadership skills and abilities.

  • Advanced Clinical Skills

Respond to and participate in opportunities to demonstrate advanced clinical skills and knowledge, incorporating evidence-based practice and research into clinical practice.

Requirements
  • Graduate of an accredited School of Nursing.
  • Bachelor of Science degree in nursing from a college/university accredited by the NLN is preferred.
  • One to two (1-2) years of medical/surgical nursing experience is preferred.
  • Ability to write, speak, and understand English, with basic nursing competencies.
Work Schedule

Monday to Friday, 8:00 AM to 4:30 PM, with occasional evening shifts and every fourth weekend, and three holidays per year.
