We are seeking a skilled Community Health Nurse to join our team at South Shore Health. As a Community Health Nurse, you will provide high-quality nursing care to patients in their homes, promoting health, preventing illness, and improving quality of life.
Key Responsibilities- Schedule Management: Complete weekly schedules and make scheduled home visits per assignment, prioritizing nursing visits according to client acuity and organizational emergency preparedness plans.
- Assessment and Documentation: Develop individualized and holistic plans of care for patients, performing admission physical and psychosocial assessments to determine appropriate care.
- Reassessment and Visit Documentation: Implement plans of care, nursing interventions, and procedures necessary for skilled patient care, updating plans as goals are met and problems are identified.
- Discharge and Documentation: Ensure patients are prepared for discharge, with all disciplines involved in care agreeing with the plan, and document improvements in outcomes or variances.
- Medication Management: Demonstrate knowledge of department policies and procedures for administering and documenting medication, including reconciliation and medication profile updates.
- Case Management: Coordinate plans of care for patients, involving patients and families in the planning process and communicating with all disciplines to ensure quality care.
- Safety Awareness: Foster a culture of safety, complying with current hand hygiene and infection control guidelines.
- Professional Development: Respond to opportunities to maintain, enhance, and promote the image of the professional nurse.
- Leadership Development: Serve in formal or informal leadership roles.
- Advanced Clinical Skills: Respond to opportunities to demonstrate advanced clinical skills and knowledge, assuming overall responsibility for professional development.
- Graduate of an accredited School of Nursing.
- Bachelor of Science degree in nursing from a college/university accredited by the NLN is preferred.
- One to two (1-2) years of medical/surgical nursing experience is preferred.
- Write, speak, understand English. Basic Nursing competencies.
