Job Overview

Position Summary

The Healthcare Support Specialist, operating across various hospital units and departments, is accountable for providing essential assistance in patient care, maintaining accurate documentation, and managing supplies. This role ensures that the unit remains well-stocked, sanitized, and orderly. Additionally, the specialist may take on administrative duties, such as answering phone inquiries, addressing questions from patients or their families, and performing data entry tasks.

In the discharge unit, the specialist plays a crucial role in facilitating the discharge process, coordinating logistics, and helping patients access necessary health resources.

This position adheres to all policies and procedures set forth by Northern Arizona Healthcare Corporation. The specialist will engage in procedures within their professional scope and collaborate closely with the interdisciplinary team. Care may be provided to a diverse patient population, including infants, children, adolescents, adults, and seniors.

The role may require flexibility, as the specialist may be assigned to various units within the hospital as needed, and on-call duties may be expected.


Key Responsibilities

Patient Care
* Ensure cleanliness within the department, adhering to protocols for post-treatment clean-up.

Assist patients with mobility restrictions and transfers to or from wheelchairs as necessary.

Support patients in wearing or removing compression garments as required.

Monitor patients for skin integrity and report any concerns to supervising healthcare professionals.

Assist with the removal of catheters and IVs, as well as phlebotomy tasks within the designated scope of practice.

Effectively prioritize and address changes in patient conditions.

Collaborate with physicians and healthcare team members to ensure appropriate staffing for quality patient care.

Quickly assess patient care needs with accuracy.

Serve as a resource for colleagues, assisting in patient care and supporting physicians with electronic medical record (EMR) documentation.

Help with dressing changes as directed by supervising clinicians.

Assist patients with daily living activities, including toileting, as needed.

Support patient mobility under the guidance of supervising clinicians, including therapeutic exercises.

Adhere to safety guidelines regarding isolation precautions and bloodborne pathogen policies.


Discharge Support
* Aid patients in transportation to and from vehicles as necessary.

Reinforce and clarify discharge instructions provided to patients.

Collaborate with the interdisciplinary team and community organizations.

Utilize the EMR for documentation and communication effectively.

Assist in securing durable medical equipment (DME) as required.

Administrative Duties
* Manage incoming administrative calls, ensuring effective communication and accurate information gathering to address caller needs.

Participate in the orientation and training of new staff members.

Promote best practices among colleagues.

Maintain unit documentation and assist with chart management as requested.

Support the charge nurse with clerical responsibilities.

Document objective patient data and care provided, including reporting abnormal findings to supervising clinicians.


Education and Compliance
* Demonstrate a thorough understanding of the unit's standards of care and administrative policies, and locate the Policy and Procedures manual as needed.

Focus on personal development and engage in professional interactions with peers and students.

Utilize nursing process guidelines in accordance with Arizona State Board of Nursing regulations, performing competently.


Safety and Compliance
* Report safety incidents promptly through the designated reporting tool; participate in safety training programs; perform duties safely; monitor the work environment for safety issues and ensure compliance among colleagues.

Stay informed and adhere to state and federal regulations and company policies affecting the role.

Maintain up-to-date certifications and licenses as required for the position.

Complete all mandatory training modules and job-specific training within the required timeframe.


Qualifications

Education

High School Diploma or GED - Required

Completion of a Nursing Assistant Program - Required

Certification & Licensure

Current, unrestricted CNA or LNA license in Arizona - Required

Basic Life Support (BLS) certification - Required

Fingerprint Clearance:
If not holding an Arizona LNA license:
- Fingerprint Clearance Card application number - Required upon hire
- Fingerprint Clearance Card - Required within 90 days of hire

Maintain appropriate clinical competencies as assigned by departmental leadership.


Experience
For Discharge Support Role:
- Demonstrated ability to collaborate with interdisciplinary teams and community organizations in patient care and management.

- Proficient in using EMR for documentation and communication.

- Knowledge of available community resources.

- Understanding of post-discharge resource planning.
