Job Summary

We are seeking a Medical Case Manager to join our team at Rehabilitation Care Coordination. As a key member of our team, you will be responsible for coordinating patient care services and ensuring seamless transitions between healthcare providers.

This is a part-time per diem position that requires strong organizational skills and effective communication abilities. You will need to drive to appointments and for patient visits as needed. A California Nursing License and Bachelor's degree in Nursing are required.

The ideal candidate will have at least 3 years of acute nursing experience, with case management experience preferred. You will be a self-starter with excellent written and verbal communication skills and able to work effectively independently and as part of a team.

In this role, you will assess patient needs, develop personalized care plans, and coordinate resources and benefits available through various government, community, and local educational institutions.

The salary for this position is $90-$110 per hour, plus mileage reimbursement. If you are a motivated and organized healthcare professional who is passionate about patient care, we encourage you to apply.
