Job Summary

We are seeking a skilled and compassionate Nurse Practitioner to join our team in Sacramento, CA. As a key member of our clinical care team, you will provide high-quality patient care and work collaboratively with our integrated care team to deliver exceptional patient outcomes.

Responsibilities
  • Provide patient-centered care to patients with complex needs
  • Collaborate with our integrated care team to develop and implement comprehensive care plans
  • Conduct home visits as needed to provide personalized care to patients
  • Work closely with our DaVita team to provide superior care for our dialysis patient population
Requirements
  • Current license to practice in the state of California
  • Current DEA license required or eligible to obtain within 90 days of hire
  • Current CPR or BLS certification
  • Minimum of two years' experience as a Nurse Practitioner
  • Preferred experience as a Family Nurse Practitioner, Primary Care Nurse Practitioner, Hospitalist, or experience in internal medicine, cardiology, Nephrology, or Case Management
