**Job Summary**

Community Healthcare Network is seeking a highly skilled and dedicated Nurse Practitioner/LPN II to join our team. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and support to our health providers in addressing the healthcare needs of our patients.

**Responsibilities**

  • Collaborate with the healthcare team to ensure the highest quality of healthcare services for patients.
  • Administer medications and injections accurately, observing patient response and documenting in the medical record.
  • Conduct patient intakes and perform venipuncture as necessary.
  • Order medications, vaccines, and medical supplies as needed.
  • Conduct regular checks on emergency equipment and refrigerator temperatures.
  • Supervise referrals and follow up on patient medical care, including correspondence.
  • Assist providers with procedures and provide health counseling to patients.
  • Confer with physicians/providers to interpret and clarify prescribed medical regimens.
  • Update prenatal log books and follow up on abnormal labs as necessary.
  • Maintain nursing competence through ongoing education and professional development.
  • Ensure confidentiality and privacy of patient information.
  • Comply with Infection Control policies and procedures.
  • Work assigned shifts at various locations as needed.
  • Perform other related duties as assigned.

**Requirements**

  • High School Diploma or equivalent.
  • Current New York State License to practice as a LPN.
  • Current CPR Certificate or plan to obtain within six months of employment.
