Job Title: Licensed Clinical Care Manager

AIDS Healthcare Foundation is seeking a highly skilled and compassionate Licensed Clinical Care Manager to join our team. As a key member of our healthcare team, you will work collaboratively with patients, physicians, and healthcare center staff to address the complex needs of our clients.

Job Summary:

The Licensed Clinical Care Manager will utilize their mental health expertise to provide comprehensive care management services to patients with HIV/AIDS. This includes conducting comprehensive needs assessments, developing individualized care plans, and facilitating access to medical and psychosocial services. The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.

Responsibilities:
  • Conduct comprehensive needs assessments to identify patient needs and develop individualized care plans
  • Collaborate with patients, physicians, and healthcare center staff to ensure seamless care coordination
  • Facilitate access to medical and psychosocial services, including referrals to community resources
  • Provide education and support to patients and their families to promote self-management and adherence to treatment plans
  • Monitor patient progress and adjust care plans as needed
Requirements:
  • Current and good standing licensure as a Bachelor or Master's Social Worker in the jurisdiction in which services are rendered
  • LMSW or LCSW required
  • Minimum of 1 year of case management experience preferred
  • Knowledge of Ryan White Services preferred
