Job Summary

Care Alliance seeks a skilled Licensed Practical Nurse to join our Outreach Team, providing supportive medical care to clients who are homeless or living in Public Housing. As a key member of our health care team, you will participate in primary care clinics at Care Alliance sites throughout the service area, fostering a collaborative approach to client health management.

Key Responsibilities
  • Deliver high-quality nursing care, including venipuncture, vital sign monitoring, and medical information recording.
  • Exhibit cultural sensitivity and a willingness to work with diverse populations, including homeless and low-income individuals.
  • Collaborate effectively with community service agency case managers and other health care professionals.
  • Perform triage/intake procedures, expediting patient flow and daily schedules.
  • Conduct basic health assessments, screenings, and report findings to the Registered Nurse and/or health care provider.
  • Implement plan of care components under the direction of the health care team.
  • Assist Podiatrist with office procedures and surgeries, maintaining sterile techniques and setting up sterile fields.
  • Schedule, review, and monitor referrals placed by Podiatry Provider.
  • Contribute to the development of nursing protocols and assist with the orientation of new clinical personnel.
  • Maintain a safe, secure, and healthy work environment, adhering to legal, HRSA, and OSHA regulations.
  • Collaborate with the health care team to develop, implement, and evaluate clinical programs.
Requirements
  • Current license to practice as an LPN in the State of Ohio.
  • Excellent verbal and written communication skills.
  • Strong problem-solving abilities.
Technology and Physical Requirements
  • Familiarity with Office 365/Outlook and Epic systems.
  • Ability to work in a normal medical/office environment, with frequent contact with staff and patients.
  • Normal range of hearing and eyesight to record or prepare reports.
