Job Title: NEST Outreach Case Manager

We are seeking a highly motivated and compassionate Outreach Case Manager to join our team at White Bird Clinic. As a key member of our outreach team, you will work closely with individuals experiencing homelessness, providing essential services and support to help them achieve stable permanent housing.

Key Responsibilities:

  • Carry a client load of outreach clients, providing assistance, advocacy, and referrals to unhoused individuals
  • Perform client assessments and assist with goal development
  • Complete Front Door Assessor training to perform FDAs in the field
  • Provide outreach and case management in the field, helping clients engage with community resources
  • Track all encounters in HMIS street outreach, including entry and exit, UDS, and EHR
  • Track and record statistics for required reports
  • Load and maintain the outreach van, ensuring it is clean and in good working order
  • Attend program meetings and outreach case conferencing meetings
  • Participate in New Bird Training

Requirements:

  • At least 2 years' experience in a crisis intervention, mental health services, substance abuse services, or related field
  • Ability to work comfortably and effectively with diverse populations, especially the unhoused
  • Possess a valid Oregon driver's license or reliable transportation
  • Must be able to lift 50 pounds
  • Able to stand for extended periods and walk up to 2.5 miles in a day
  • Ability to network with other agencies and assist with Government resources
  • Ability to work on the street and in other non-traditional environments, as well as in an office setting

Preferred Qualifications:

  • Bilingual/Spanish preferred

Compensation:

This is a non-exempt position with a target starting base compensation of $18- per hour paid on a bi-weekly basis.
