Housing Navigator Job Description

At Equitable Social Solutions, we are seeking a highly skilled and compassionate Housing Navigator to join our team. As a Housing Navigator, you will play a critical role in empowering individuals and families to achieve long-term stability and success.

Key Responsibilities:

  • Establish and maintain effective relationships with clients, community partners, and stakeholders to provide comprehensive support services.
  • Conduct in-depth assessments to identify client needs and develop personalized action plans to address housing and stability challenges.
  • Collaborate with clients to identify and overcome barriers to housing and stability, providing guidance and support to achieve self-sufficiency.
  • Manage and maintain accurate records and files, ensuring confidentiality and compliance with regulations.
  • Develop and implement effective strategies to promote permanent housing opportunities and support services for homeless individuals and families.
  • Provide individual and group counseling, referrals, and support services to clients, as needed.
  • Work closely with community partners to promote outreach and education, ensuring effective communication and collaboration.
  • Perform other duties as assigned to support the mission and goals of Equitable Social Solutions.

Requirements:

  • Bachelor's Degree in Human Services or related field, or equivalent experience.
  • Intermediate computer skills in Microsoft Office, Email, and Data Entry.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Valid driver's license and good driving record.
  • Ability to travel throughout the County, as needed.
