Job Summary

We are seeking a highly organized and detail-oriented Case Manager to join our human services team. The successful candidate will play a crucial role in coordinating comprehensive support for our students, helping them achieve their individual goals. As a Case Manager, you will act as a liaison, advocate, and coordinator, facilitating student engagement in support groups, vocational programs, transportation services, community activities, and independent living skills development.

Key Responsibilities
  • Coordinate comprehensive support for students, helping them achieve their individual goals
  • Act as a liaison, advocate, and coordinator, facilitating student engagement in support groups, vocational programs, transportation services, community activities, and independent living skills development
  • Foster open, supportive communication between students, staff, families, and community organizations, ensuring each student's needs are met holistically
  • Oversee staff development, providing training and mentorship to ensure they effectively support our students
  • Document progress, manage medical care and communication, and ensure all support services are properly implemented
Requirements
  • Bachelor's degree in a relevant field (e.g., Social Work, Psychology, Human Services) is preferred. Master's degree is a plus.
  • Proven experience in case management or related roles, with a strong track record of achieving positive outcomes for clients.
  • Excellent communication, interpersonal, and collaboration skills.
  • Knowledge of relevant ethical principles and industry guidelines.
  • Ability to maintain confidentiality and adhere to legal and regulatory standards.
  • Strong organizational and time-management skills.
  • Commitment to continuous professional development and a passion for advancing the field of case management.
  • First aid and CPR certification or the ability to obtain certification within the first 30 days.
  • A basic understanding of ASD (autism spectrum disorder) and some of the supports and accommodations for it.
  • The patience, empathy, and understanding to work with adults who have ASD (autism spectrum disorder).
  • Ability to pass a background check upon hire.
  • Driver's license and a clean driving record.
