MDS / RN Care Coordinator

POSITION SUMMARY

The MDS / RN Care Coordinator plays a crucial role in managing the RAI process, ensuring compliance with both federal and state regulations, and working closely with nursing personnel to execute care strategies. This full-time role is under the supervision of the facility administrator.

KEY RESPONSIBILITIES

  • Oversee the RAI process and guarantee the prompt completion of MDS, CAAs, and care strategies
  • Facilitate care plan discussions with residents and their families
  • Ensure care plans are current and accurately maintained
  • Review MDS submissions for precision and identify educational trends
  • Monitor documentation for residents under Medicare and managed care
  • Engage in the Quality Improvement initiatives
  • Provide guidance on the financial aspects related to RUGS, PPS, and case mix rates
  • Keep updated with the latest regulations through ongoing training and seminars
REQUIREMENTS
  • RN degree from an accredited institution
  • AANAC certification is preferred
  • Current CPR certification
  • A minimum of 2-4 years of experience in MDS and long-term care settings
  • Familiarity with Point Click Care is advantageous
