MDS Coordinator

Job Summary:
The MDS / RN Coordinator is responsible for overseeing the RAI process, ensuring schedules are followed, and timely completion of the MDS, CAAs, and care plans in accordance with federal and state guidelines. This is a full-time position that reports to the administrator of the facility.
Key Responsibilities:
- Collaborate with nursing staff to implement care plans
- Oversee care plan meetings with residents and families
- Ensure care plans are up-to-date and reflective of resident needs
- Verify assessments are transmitted to the database with necessary corrections
- Provide education to staff on ADLs, MDS, and other relevant areas
- Review MDS for accuracy and identify trends that require education
- Monitor documentation of Medicare and managed care residents, and educate as needed
- Focus on quality metrics, including those impacting five-star ratings
- Experience with CMI for optimal outcomes and documentation guidelines
- Knowledge of restorative programs and implementation
- Participate in Quality Improvement programs in the facility
- Provide leadership on financial implications of RUGS, PPS, and case mix rates
- Minimum 2-4 years of experience in MDS and long-term care
- Point Click Care experience preferred
- Willingness to attend seminars to stay current with regulations
Requirements:
- RN degree from an accredited college (minimum ASN)
- Current registration in the state
- AANAC certification
- Current CPR certification
