About the Role:
The Patient Care Coordinator will be responsible for providing medical triage, maintaining a caseload of lower acuity patients, documenting phone calls and patient interactions, and assisting with medication refill requests.
Key Responsibilities:
- Provide medical triage for inbound clinical inquiries via phone calls and patient portal messages
- Maintain a paneled caseload of lower acuity patients
- Document all phone calls and patient interactions in the EHR
- Assist with patients' medication refill requests
Required Knowledge and Skills:
- Ability to react decisively and quickly in emergencies
- Knowledge of medications and their effects
- Experience with multi-tasking
- Proficiency in using electronic health records and Microsoft Office Suite
