Job Summary

Special Service for Groups, Inc- HOPICS is seeking a skilled Case Manager to join our team. As a Case Manager, you will be responsible for providing comprehensive case management services to individuals, families, and youth seeking services through the Coordinated Entry System. Your primary goal will be to facilitate clients' successful transition to permanent housing and coordinate services to foster housing stability during and beyond the Shallow Program subsidy.

Key Responsibilities
  • Coordinate with other project staff and partners to provide housing services and supportive services to clients.
  • Update clients' housing status within 24 hours of any change.
  • Track services, referrals, and case notes, and complete them within 24 hours following service provision.
  • Ensure all information is uploaded into HMIS within 24 hours.
  • Assist with Shallow Subsidy referrals and Status Assessment Updates.
  • Enter data into HMIS within 24 hours.
  • Prepare project reports in accordance with funding requirements.
  • Provide training and support to necessary staff, providers, and project partners.
  • Maintain accurate documentation in client files and provide required client information and assessment outcomes.

Requirements
  • Associate degree in social services or a related field.
  • Minimum 1-2 years of job-related experience working with homeless individuals and families.
  • Case management and homeless program experience preferred.
  • Working knowledge of Microsoft Word, Excel, and other database programs.
  • Ability to work with diverse cultural, ethnic, and socio-economic backgrounds.
  • Ability to communicate effectively, both written and orally.
  • Verification of Employment Eligibility and Background Clearance.
  • TB test required every year or as needed.
  • CPR and First Aid Certification required every two years or as needed.

Working Conditions
  • This position requires working in client-friendly environments and visiting other shelters and homeless access centers.
  • Local automobile travel is required.
  • Some responsibility to work in noisy environments where children and adults are present.

Physical Requirements
  • The Case Manager typically spends time sitting, standing, typing, thinking, writing, walking, driving, carrying (max. 25 lbs), listening, speaking.

Mental Requirements
  • This position requires handling constant distractions, interruptions, and uncontrollable changes in priorities/work schedules.
  • Ability to process information, think, and conceptualize.
