Associate Manager of Case Management

Job Summary:
The Associate Manager of Case Management is a key leadership position responsible for overseeing the recuperative care and short-term post-hospitalization processes at Illumination Foundation. This role reports to the Manager of Case Management and is responsible for supervising case management staff, ensuring contract deliverables are met, and developing external relationships with other service providers.
Key Responsibilities:
- Supervise case management staff, providing guidance and support to ensure effective case planning and client care.
- Coordinate the hiring process for new case management staff and ensure a smooth onboarding experience.
- Work closely with Site Managers to make client decisions, including exits, extensions, intakes, and other case planning action steps.
- Develop and implement training programs to improve staff skills and client care.
- Provide case consultation and emergency management to support case managers and clients in creating crisis management plans.
- Oversee care coordination with Program Managers and Site Managers to ensure client care, effective case planning, and positive outcomes.
- Ensure accuracy of data for contract compliance, grant reports, and other reporting needs.
Requirements:
- Bachelor's degree in Social Services or a related field.
- Minimum 2 years of recent progressively responsible administrative management/supervisory experience.
- Previous experience working as a Case Manager or similar position.
- Experience in supervision or project management and oversight.
- Current valid CA driver's license and a dependable vehicle with automobile insurance.
- Computer literate, with the ability to use Microsoft Office products and databases such as Homelessness Management Information Systems (HMIS).
Preferred Qualifications:
- Masters Degree in Social Services and 2 years of experience in the Social Services field.
- At least 2 years of experience working with at-risk/homeless individuals.
- Bilingual in Spanish and English is highly desirable.
