About the Opportunity:
We are seeking a detail-oriented and organized RN Care Manager-Resource to join our team at NorthShore University HealthSystem. As a key member of our healthcare team, you will play a vital role in ensuring seamless patient care and navigation through the healthcare system.
Job Summary:
The RN Care Manager-Resource will be responsible for facilitating the collaborative management of patient care across the continuum, intervening as necessary to remove barriers to timely and efficient care delivery and reimbursement.
Key Responsibilities:
- Facilitate the collaborative management of patient care across the continuum.
- Intervene as necessary to remove barriers to timely and efficient care delivery and reimbursement.
- Work collaboratively and maintain active communication with physicians, nursing, and other members of the multidisciplinary care team to effect timely, appropriate patient progression.
- Monitors the patient's progress, intervening as necessary and appropriate to ensure that the plan of care and services provided are patient-focused, high-quality, efficient, and cost-effective.
Requirements:
- Bachelor's degree in Nursing from an NLN accredited school of nursing is preferred.
- Minimum 2 years of clinical nursing experience preferred.
- Knowledge of InterQual or MCG criteria is preferred.
- Clinical certification, such as case management certification, is beneficial.
- Strong computer and data entry skills.
- Experience with Electronic Medical Record (EMR) platform preferred.
- Proven leadership skills.

The salary range for this position is approximately $85,000 - $115,000 per year, based on experience and qualifications. We offer a competitive benefits package, including health insurance, retirement plan, and paid time off.
