Job Responsibilities
As a Patient Care Coordinator in our Healthcare Services department, you will play a critical role in ensuring seamless coordination of patient care across the continuum. Your duties will include facilitating the collaborative management of patient care, intervening as necessary to remove barriers to timely and efficient care delivery and reimbursement. You will work closely with the CDI team to ensure that documentation accurately reflects severity of illness and intensity of service.

The ideal candidate will possess a strong understanding of clinical decision-making, implementation of recommendations, and discharge planning. They will be able to communicate effectively with healthcare teams and stakeholders, and have experience with Electronic Medical Record (EMR) platforms. A Bachelor's degree in Nursing from an NLN accredited school of nursing and a minimum of 2 years clinical nursing experience are required for this role.
