Responsibilities
  • The Senior Care Manager is responsible for providing overall leadership and management to the Assisted Living, Terrace Club, and/or Reminiscence Neighborhoods assigned.
  • This includes participating in the evaluation and service planning of residents, leading the care manager team in providing high-quality safe care to residents, collaborating with the Resident Care Director when residents require care of a clinical nature, and creating staffing scheduling plans for the care manager team.
  • The Senior Care Manager assists in recruiting, hiring, training, and coaching team members, and is responsible for team member recognition, engagement, and performance management evaluations.
About this Role

This role is a fundamental part of the Sunrise team, supporting the Mission Principles and Service Core Values. As a Senior Care Manager, you will plan, organize, develop, and lead the overall operation of the Assisted Living, Terrace Club, and/or Reminiscence Neighborhood(s) in accordance with federal, state/provincial, and local laws, as well as Sunrise Senior Living standards, guidelines, and policies.

Key Responsibilities:
  1. Participate in resident evaluation and contribute to the development of resident Service Plans (SP).
  2. Maintain appropriate staffing levels and create daily assignments based on resident evaluations and assigned levels of care.
  3. Schedule and lead move-in meetings with new residents and their families to discuss resident care needs and preferences, and proposed Service Plan; coordinate with the Resident Care Director when clinical care or training needs arise.
  4. Recognize changes in resident condition and take appropriate action, including making notifications.
  5. Access and use documents, actions, and Sunrise systems, including electronic health records, Sunrise Care Connect, iCIMS, and OnShift, etc.
  6. Participate in community Interdisciplinary Team (IDT) meetings to review and discuss selected residents' changes in care needs, Service Plan interventions, and other relevant topics.
  7. Partner with Activities Volunteer Coordinator (AVC) to schedule neighborhood activities and ensure resident participation, and facilitate care manager involvement in activities and staffing.
  8. Partner with Dining Services Coordinator (DSC) for special events and monitor resident dining experiences.
  9. Engage in strong, proactive communication with families on a monthly basis, minimum, to provide information on changes in condition, updates on resident life, and community happenings.
  10. Communicate with physicians and support services as appropriate.
  11. Responsible for developing resident schedules for bathing, laundry, housekeeping services, etc.
  12. Conduct monthly Lead Care Manager Meetings.
Quality Assurance Regulatory Compliance:
  1. Follow federal, state/provincial, and local laws and regulations related to resident care services.
  2. Strive for excellence in quality care and service delivery, participating in community QAPI meetings, Performance Improvement Plan committees, and required regulatory compliance.
  3. Follow approved plans for correction and ensure ongoing regulatory compliance.
  4. Maintain thorough knowledge of current evolving state laws, regulations, policies, and procedures related to resident care and community operations, ensuring compliance.
  5. Partner with the leadership team to ensure community compliance with OSHA requirements.
  6. Assist leadership in promoting Sunrise policies and programs related to resident care, quality, safety, and risk management.
Financial Management:
  1. Explain and discuss the value of Sunrise products and services to residents, families, and team members, targeting referral sources.
  2. Assist the Executive Director in completing the annual community budget.
  3. Understand and manage department budgets, labor expenses, and other expenses, discussing the impact on the overall community budget.
  4. Review monthly financial information, implement plans for action, and address deficiencies.
  5. Process and submit monthly expenses and budget data in a timely manner, following Sunrise policies and internal business controls.
  6. Coordinate the community team to achieve maximum staff economies through cross-training and other initiatives.
  7. Understand internal costs associated with Sunrise resident care programs, such as care levels, medication programs, and continence products.
Training Leadership Team Member Development:
  1. Perform overall management of the department, including but not limited to recruiting, hiring, training, coaching, and performance improvement.
  2. Provide training to care manager staff on job responsibilities, resident activities, and daily living.
  3. Partner with delivery of Sunrise University onboarding and annual training as needed.
  4. Achieve stated Team Member Engagement goals and actively lead engagement improvement initiatives as needed.
  5. Complete team member staffing and scheduling according to operational and budgetary guidelines.
  6. Review timekeeping practices and payroll reports to ensure hours worked are recorded appropriately and paid.
  7. Conduct timely performance appraisals and meaningful conversations.
  8. Hold team members accountable for correcting actions necessary, documenting conversations and actions taken.
  9. Attend regular community meetings, including Stand Up, Cross Over Department Head, Town Hall, QAPI, IDT, and others required by the Executive Director.
  10. Keep abreast of professional developments in the field, reading, attending conferences, and training.
  11. Maintain compliance with assigned required training applicable to the role, ensuring Sunrise standards are met.
Qualifications:

To perform this job successfully, the individual must be able to perform each essential duty satisfactorily. The requirements listed throughout the job description represent knowledge, skills, abilities, and experience required.

The ideal candidate should have one year of experience in assisted living, long-term care, or hospital industries, with exposure to memory-impaired senior populations preferred. One year of supervisory management experience, including hiring staff, coaching, performance management, daily operations supervision, discipline, counseling, is also preferred. A college degree and management experience may be required per state regulations.

The successful candidate will have the ability to manage multiple priorities, delegate assignments to appropriate individuals based on their skills and roles, possess written and verbal skills for effective communication, facilitate small group presentations, demonstrate competent organizational and time-management skills, good judgment, problem-solving, and decision-making skills.

Demonstrating proficiency in computer skills, Microsoft Office, Windows, Outlook, Excel, and Sunrise applications, with the ability to learn new applications, is essential. Ability to work weekends, evenings, and flexible hours, available to customers during peak service delivery days and times.

Benefits & Compensation

This role offers a competitive salary range of $65,000 - $80,000 per year, depending on location and experience, plus a comprehensive benefits package, including:
