Position Overview:

The Familial Support Coordinator is an integral member of the Treatment team at WestCare Foundation, dedicated to delivering exceptional services to individuals participating in the Health Resources and Services Administration (HRSA) grant.

This role encompasses the provision of individual, group, and family counseling services, along with comprehensive assessments and interventions tailored to the needs of families and individuals. The Familial Support Coordinator will operate within designated regions, ensuring effective outreach and linkage services for eligible participants.


Key Responsibilities:


1. Maintain accurate clinical documentation, including the creation of individual service plans, daily progress notes, and monthly evaluations.

2. Conduct thorough interviews to assess the needs of individuals and families, determining suitability for counseling and treatment programs.

3. Schedule and facilitate intake appointments and assessments for eligible participants.

4. Collaborate with the Tennessee Department of Children Services (DCS) to ensure equitable access to services and programs.

5. Coordinate with community resources to streamline care and support for participants.

6. Review and analyze treatment plans and assessments related to substance use disorders and co-occurring conditions.

7. Provide crisis intervention and report incidents to the appropriate supervisory staff.

8. Lead individual, group, and family counseling sessions utilizing evidence-based practices such as Cognitive Behavioral Therapy (CBT).

9. Participate in regular treatment team meetings and contribute to the continuous improvement of service delivery.

10. Uphold the mission and values of WestCare Foundation, ensuring high standards of ethical conduct and confidentiality.


Qualifications:


1. A degree in a relevant field such as Communication, Sociology, Social Work, or Psychology is preferred.

2. A minimum of 3-5 years of experience in a clinical or related setting.

3. Previous experience working with individuals facing substance use disorders is advantageous.

4. Strong communication skills, both verbal and written, are essential.

5. Ability to work collaboratively within a team and maintain professional relationships with community organizations.

6. Proficiency in utilizing technology and data tracking systems to support program outcomes.


Work Environment:

This position primarily operates in an office setting, with occasional travel required for training and community outreach activities. Regular attendance and punctuality are essential for success in this role.
