Job Overview

Felton Institute is seeking a qualified Clinical Outreach Specialist to join our team. This role will work with clients experiencing homelessness, substance use, and mental health challenges.

Salary: $92,500 - $108,000 per year

Responsibilities
  • Provide trauma-informed, culturally responsive services to clients, including mental health services, harm reduction, contingency management, and motivational interviewing.
  • Elicit and maintain strong working relationships with clients and the care team, which may include other professionals, peers, medical providers, and client-identified support persons.
  • Facilitate and track linkages, care coordination, and case management among medical and community-based supports.
  • Outreach to shelters, streets, hospitals, and community settings to meet clients where they are and locate them using electronic health record systems and planning with the client.
Requirements
  • Master's Degree in Social Sciences, Psychology, Counseling Psychology, or a related field, AND current California State Licensure or registration as an associate.
  • Two years of experience providing mental health services to individuals with serious mental health challenges, substance use, and homelessness.
  • Experience with specific modalities, harm reduction, contingency management, substance use disorder treatment, crisis intervention, de-escalation, and motivational interviewing.
  • Demonstrated proficiency in Microsoft applications, phone-based applications, and navigating EHR systems (EPIC and CIRCE experience preferred).
  • Bilingual in threshold languages (Spanish, Cantonese, Mandarin, Vietnamese, Russian, Tagalog) required.
