Key Responsibilities- Coordinate patient services with nursing agencies and monitor patient progress toward goals
- Educate patients, caregivers, staff, and other professionals regarding appropriate medication use
- Determine the suitability of individual patients for home care and collect and organize patient-specific information needed to determine suitability
- Assess patients' suitability for home care in accordance with Ambrosia Healthcare inc policies
- Design, recommend, monitor, and evaluate patient-specific pharmacy care plans for the pharmacotherapy of home care patients
- Make suitable recommendations for modifications in prescribed medication therapy or monitoring parameters for home care patients
- Collect and organize patient-specific information needed to prevent, detect, and resolve medication-related problems and make appropriate medication therapy recommendations
- Identify appropriate monitoring parameters for each patient
- Complete nutritional assessments of patients and make appropriate recommendations
- Modify pharmacy care plans for home care patients based on evaluation of monitoring data and other pertinent patient-specific information
- Be competent in the use of pharmacokinetic principles and formulas and assessing patient laboratory values
- Provide medication-use education to Ambrosia Healthcare inc staff, other professionals, home care patients, and/or their caregivers
- Use effective patient education techniques to provide counseling to home care patients and/or their caregivers
- Ensure continuity of pharmaceutical care to and from the home and other patient-care settings
- Use a systematic procedure to communicate pertinent patient information to and from the home and other patient-care settings
- Prepare and dispense medications using appropriate techniques and following Ambrosia Healthcare inc policies and procedures
- Accurately calibrate equipment and prepare medications so they are appropriately concentrated, without incompatibilities, stable, and appropriately stored
- Adhere to appropriate safety and quality assurance practices
- Prepare labels that conform to the organization's policies and procedures and ensure that medication contains all necessary and/or appropriate ancillary labels
- Inspect the final medication before dispensing and assure presence of authorization to dispense
- Comply with state/federal laws and USP standards, including DEA controlled substance regulations
- Comply with policies regarding the perpetual inventory systems for schedule II narcotics, controlled substances, and other medications subject to diversion
- Manage the use, maintenance, and troubleshooting of medication administration equipment and medication-related equipment used in the management of home care patients
- Accurately program, maintain, and give advice to others on the use and maintenance of all medication administration and medication-related equipment used by Ambrosia Healthcare inc
- Participate in the process for assessing, managing, and reporting medication-related incidents and equipment error and/or failures
- Accurately complete occurrence forms and document according to Ambrosia Healthcare inc policy
- Monitor and report discrepancies in perpetual inventory systems for schedule II narcotics and other medications subject to diversion
- Provide concise, applicable, and timely responses to requests for drug information from healthcare providers and home care patients
- Formulate responses to drug information requests based on analysis of the literature
- Attend educational seminars or participate in other CEU opportunities to promote professional competency and comply with state-mandated CEU requirements
- Manage the use of investigational drug products according to established regulations and Ambrosia Healthcare inc policies and procedures
- Utilize pharmacy support personnel effectively and prioritize the workload and organize the workflow, taking into account available resources
- Ensure the accuracy of the work of pharmacy support personnel and comply with accreditation, legal, regulatory, and safety requirements
- Maintain confidentiality of patient and proprietary information and observe legal and ethical guidelines for safeguarding the confidentiality of patient and proprietary Ambrosia Healthcare inc information
- Participate in the Performance Improvement committee and be delegated specific tasks to trend and report to the committee
- Manage and organize the ongoing functions of Ambrosia Healthcare inc to ensure that day-to-day operations are not hampered in the absence of the Pharmacy manager
- Be responsible for the ongoing liaison among the governing body/owner and personnel
- Monitor employee competency and compliance with regards Ambrosia Healthcare inc policies and procedures
- Review and implement effective budgets and accounting to maintain the daily operations
- Graduate of an accredited School of Pharmacy
- Required licensure in Pharmacy as granted by the Board of Pharmacy
- At least 1 year of experience with knowledge of sterile IV admixture procedures or equivalent knowledge of non-sterile and sterile compounding, immunizations, and MTM
- Experience providing customer service to internal and external customers, including meeting quality standards for services, and evaluation of customer satisfaction
- Basic level skill in Microsoft Word, Excel, and PowerPoint
- Have a reliable means for transport from/to work during the week, including potential on-call during the weekend, and an active driver's license is required
- Completion of a Post Graduate residency program experience in home care or hospital setting
- Experience in compounding of sterile and non-sterile products and current working knowledge of USP 797/800 standards
- Working and up-to-date knowledge of hormone replacement therapies
- Experience with monitoring, adjusting, and making recommendations to providers with regards to IV nutrients
- Comfortable with osmolarity, tonicity, and dilution calculations per providers' request
- At least 1 year of experience in indirect management of team members, including assisting in the development, training, and assignment of work/projects to other team members
- Organizational skills are KEY for this busy Infusion Pharmacy
- Solid understanding of the Infusion business to include compounding
- Basic mathematic skills for calculating dosage/concentration
- Social skills to interact with patients and other healthcare professionals
- Previous experience of 1-2 years in a Home Infusion, Hospital setting, or Compound/Retail setting
