Position Summary:
This entry-level role is pivotal in the comprehensive coordination of care for patients receiving home infusion therapy. A Senior Intake Coordinator will serve as a mentor during the initial probationary period.

High-performing Intake Coordinators who demonstrate a commitment to personal and professional development, and who build strong relationships within and outside the department, may be eligible for promotions to:
- Senior Intake Coordinator – typically within 2-5 years based on performance and engagement.
- Lead Intake Coordinator – for those who take on leadership responsibilities within the department.

Key Responsibilities:
- Compile and distribute initial Patient Profiles for therapy to the healthcare team.
- Manage incoming calls for the Intake Workgroup, ensuring proper documentation.
- Facilitate effective communication with pharmacy and delivery staff regarding patient referrals and issues.
- Collaborate with the Liaison/Care Coordination team to streamline discharge processes.
- Document all patient-related information in clinical records accurately.
- Update patient supply requirements and communicate changes to relevant teams.
- Ensure adherence to professional appearance standards.
Qualifications:
- Associate of Arts degree preferred.
- Previous experience in a medical office is advantageous.
- Familiarity with medical terminology is preferred.
